Introduction: Why Is Penis Size So Important?

A significant number of male patients who come to my office share a common concern:
“Doctor, is my penis small?”

This statement is usually a source of anxiety that undermines self-confidence, affects one’s sex life, and is often exaggerated.
Unfortunately, society’s perception of penis size has become completely detached from reality due to pornography, social media, and unfair comparisons.

For this reason, before I explain surgical penis enlargement, I always make the following point clear:

👉Whether a penis is small or not is determined not by “visual inspection” but by medical measurement.

In this blog post, I discuss surgical penis enlargement in all its aspects:

  • In what situations might it actually be necessary?
  • In what situations is it unnecessary?

  1. What Is the Average Penis Size? Let’s Start with the Scientific Basis

When assessing penis length,the length during an erectionis used as the basis.
The scientific measurement of this:

  • Through the bone (pubic bone)
  • Along the shaft of the penis
  • It is done by measuring to the very end.

📌 Scientific averages:

  • Average length when erect:12–16 cm
  • 11 cm and above:normal
  • 7–11 cm:lower limit – borderline
  • Less than 7 cm: may fall into themicropeniscategory

A flaccid state is not a measure.

It is greatly influenced by factors such as stress, temperature, weight, and psychological factors.

In my clinical practice, I have often encountered men—even those with an erect penis length of 15 cm—who come to me worried that “it’s too short.”
For this reason, my first criterion is as follows:

  1. What Is Surgical Penis Enlargement?

Surgical penis enlargement is the general term for reconstructive and cosmetic surgical procedures performed to increase the length, girth, or both of the penis.

The purpose of these surgeries is:

  • Elongation
  • Increase in thickness
  • More pronounced appearance when flaccid
  • The shaft of the penis is positioned further forward
  • Visual and functional satisfaction

Surgical penis enlargement** is not a miracle**; it should be approached with realistic goals, not exaggerated expectations.

  1. Penis Lengthening Surgery

There is a “suspensory ligament” that attaches the penis to the anterior abdominal wall:the suspensory ligament.
This ligament holds part of the penis inside the body.
The basic principle of surgical lengthening:

👉The goal is to release some of these ligaments so that the penis can extend outward more fully.

You can think of it like a tent rope; when the rope is loosened a little, the tent stretches forward.

3.1 Can the Penis Really Grow Longer?

Yes, but:

The length of the slack state varies the most.

There is usually a noticeable difference of about2–4 cm.

The length of the penis during an erection is limited.

In most patients, it is around1–2 cm.
In some patients, this makes a difference, while in others, the effect is minimal.

🔹Since the biomechanical structure of erectile rigidity has changed, it is not realistic to expect significant elongation.

3.2 For whom does it work best?

  • Men with potbellies
  • Those whose penile shaft is hidden beneath fat
  • Buried penis
  • A penis that appears noticeably short when flaccid
  • Those who are of average height but want a more pronounced appearance

3.3 Are there any risks?

Yes. As with any surgery, there are risks involved here as well:

  • A slight downward shift in the angle of the erection
  • Hardness in the healing tissue
  • Asymmetry
  • Dissatisfaction Due to Unrealistic Expectations
  • Rarely an infection
  • Wound healing problem
  • The need to use a traction device

Factors that reduce this risk include the surgeon’s experience and the management of the patient’s expectations.

  1. Penis Thickening Surgery

Thickening surgeries are procedures that can make a greater difference than lengthening.
Most patients are more dissatisfied with the thickness than with the length.

The most common techniques:

4.1 Fat Injection (Lipofilling)

  • Fat is removed from the body (abdomen, waist, thighs)
  • It is purified using specialized equipment
  • It is injected evenly into the shaft of the penis

Advantages:

✔ Natural, with its own texture
✔ Very noticeable thickness in the first few months
✔ Makes a thin penis visually satisfying

Disadvantages:

❗ Over time, some of the wax will melt
❗ Uneven melting can cause irregularities
❗ May require an additional session

For this reason, fat injection must be performed on the right patient using the right technique.

4.2 Dermal Grafts, Fat Grafts, and Synthetic Grafts

In some cases:

  • Dermal fat grafts
  • Special grafts taken from subcutaneous tissue
  • Certain medical supplies

It is surgically implanted around the penis.

These techniques may be more long-lasting, but:

  • The surgery is more difficult
  • It takes longer to heal
  • The risk of infection is higher

For this reason, it is appropriate in selected cases.

4.3 Thickening with Filler (Non-surgical)

Penis augmentation without surgery:

  • Contains hyaluronic acid
  • It takes 20–30 minutes
  • It is returnable
  • The increase in thickness takes an average of 6–24 months

This should not be confused with surgical augmentation; they are two different procedures.
For many patients, non-surgical fillers may be sufficient.

  1. Combined Techniques in Surgical Penis Enlargement

In some patients:

👉 You can combine lengthening and thickening.

This technique yields much more satisfactory results, especially in patients who are shorter than average and also underweight.

However:

  • The duration of the surgery is extended
  • The risk and recovery time increase
  • Requires an experienced surgical team
  1. Who Is a Good Candidate for Surgical Penis Enlargement?

Qualified candidates:

  • People with true short stature
  • Micropenis
  • Buried penis
  • People who have previously undergone unsuccessful surgery
  • Those who have not benefited from fillers or other methods
  • People with severe visual impairments
  • Those with realistic expectations

Ineligible:

  • People with a medically normal penis who have obsessive thoughts
  • Those who come under pressure from their partners
  • Those who compare it to pornography and develop unrealistic expectations
  • Those who set impossible goals, such as “grow 10 cm”
  • People with psychiatric disorders

Surgical penis enlargementis not a procedure performed on just anyone who wants it.

  1. What Do I Discuss During the Preoperative Consultation?

  1. Height measurement→ is performed objectively and accurately.
  2. Vascular function and erectile capacityare assessed.
  3. The fat distribution in the abdominal and pubic regionsis examined.
  4. First non-surgical methods is debated:
    • Penis augmentation
    • ESWT
    • Exosome
    • PRP
    • Hormonal regulation
  5. The patient's expectations are clarified:
    • Erection length?
    • Does it look flaccid?
    • Thickness?
  6. The limits of surgery are clearly stated.
  7. Risks are explained transparently.

I do not recommend surgery for patients who cannot pass these stages.

Risks:

  • Bleeding
  • Infection
  • Asymmetry
  • Fat necrosis (hardening of fatty tissue)
  • Change in the angle of erection
  • Recovery issues
  • Nerve damage is very rare
  • Irregularity in oil injection
  • Unsatisfactory result
  • Need for revision (correction)

Surgery is a serious decision.
That is why “transparency” with the patient is my most important ethical approach.

  1. What Is the Postoperative Process Like?

The first 48 hours:

  • Swelling, bruising, and mild pain are normal
  • A clean dressing is applied
  • Rest is important

The first week:

  • Getting up, light activities
  • Swelling decreases
  • You can return to work (depending on your profession)

4–6 weeks:

  • Sexual intercourse is prohibited
  • The stitches will heal
  • The shape begins to take form

3–6 months:

  • The final result becomes clear
  • If fat injection is performed, the results become permanent
  • If lengthening surgery is performed, the height stabilizes
  1. How Realistic Are the Surgical Results?

I often use the following table to manage expectations effectively:

This section is 100% about psychology.
For many men:

  • Feeling comfortable in front of the mirror in the shower,
  • To engage in a relationship with your partner with confidence,
  • A reduction in shame and anxiety

It is more important than one's sex life.

Surgerycan significantly improve self-confidence in the right patient.
However, in the wrong patient, it can have the exact opposite effect.
That is why a professional approach is essential.

  1. My Final Thoughts on Surgical Penis Enlargement (A Clinical Approach)

I don’t perform these types of surgerieson just anyone who asks.
Because:

  • Surgery is an irreversible step.
  • Expectations must be reasonable.
  • We should first consider the simpler methods.
  • The risks are high, and an experienced team is absolutely necessary.

But when done by the right person, using the right technique:

✔ Can make a noticeable visual difference
✔ Can boost self-confidence
✔ Provides functional benefits in cases such as a buried penis
✔ Can provide comfort in one’s sex life
