One of the most common problems I encounter in my urology practice is erectile dysfunction—commonly known as impotence or difficulty achieving an erection. Most men find it difficult to talk about this issue, but it is much more common than people realize. Its prevalence increases significantly, especially among men over the age of 40.
Erectile dysfunction is the inability to achieve or maintain an erection sufficient for sexual intercourse. This condition negatively affects not only physical health but also psychological and social well-being. For many men, “erection problems” can lead to a loss of self-confidence, communication issues between partners, and a significant decline in quality of life.
1. What Is Erectile Dysfunction? Who Is Affected?
Erectile dysfunction can occur in men of all ages, but its prevalence increases, particularly with advancing age.
•Among young men, stress, anxiety, a hectic work schedule, relationship problems, and unhealthy lifestyle habits are typically the main issues.
•Among middle-aged and older adults, atherosclerosis, diabetes, high blood pressure, high cholesterol, cardiovascular diseases, and hormonal disorders are among the most common causes.
In particular, smoking, alcohol, obesity, and a sedentary lifestyle play a significant role among the causes of erectile dysfunction.
Key Symptoms
•Inability to achieve sufficient erection for sexual intercourse
•Rapid loss of erection
•Even if an erection persists, it may not be satisfying
•The inability to achieve a normal erection in the morning or at night
These symptoms may occur a few times, but if they recur frequently, they must be evaluated. The answer to the question, “Is erectile dysfunction temporary or permanent?” depends on identifying the underlying cause.
2. What You Need to Know About Erectile Dysfunction
Impact on Quality of Life
Erectile dysfunction affects not only a person’s sex life but also their overall quality of life. Loss of self-confidence, communication problems with a partner, withdrawal from social life, and depression are common consequences.
Temporary or Permanent?
Most patients ask me this question: “Doctor, is this problem permanent?”
•If the cause is stress, fatigue, or a temporary psychological condition, the problem may resolve itself.
•However, if there is a vascular or hormonal disorder, treatment is absolutely necessary.
Common Misconceptions
•“Erectile dysfunction occurs only in older men.” (False; it can also be common in younger men.)
•“Erectile dysfunction goes away on its own.” (This isn’t always true; sometimes there may be an underlying serious medical condition.)
•“There is no treatment for this condition.” (Wrong. Today, there are very effective treatment methods.)
3. Causes of Erectile Dysfunction
Physical Causes
•Vascular diseases: This is one of the most common causes. A decrease in blood flow to the penile arteries leads to erectile dysfunction.
•Hormonal imbalances: Low testosterone is a significant factor.
•Neurological disorders: Spinal cord injuries, nervous system disorders.
•Systemic diseases: Diabetes, high blood pressure, cardiovascular diseases.
Psychological Causes
•Stress, heavy workload
•Performance anxiety
•Relationship problems
•Depression and anxiety
In many patients, these factors are present together. Therefore, an accurate diagnosis and personalized treatment planning are very important.

4. Current Treatment Options
Today, the treatment of erectile dysfunction is quite successful. For my patients, I prefer a stepwise approach—starting with simpler options and progressing to more advanced ones—while taking their individual circumstances into account.
4.1. Medication Treatments
Oral medications that cause blood vessels in the penis to dilate are the most commonly used first-line treatment. They are quite effective in the appropriate patient population.
4.2. ESWT (Shock Wave Therapy)
This is a method I have frequently used in recent years. Low-intensity shock waves are applied to the blood vessels in the penis to stimulate new blood vessel formation. The goal is to achieve lasting improvement.
4.3. Injection and Filling Methods
In patients for whom medication is not sufficient, an erection can be achieved through penile injections or filler treatments.
4.4. Penile Prosthesis Implants
For patients with advanced-stage disease or those who have not responded to other treatments, penile prosthesis surgery—also known as the “happiness rod”—offers a permanent solution. It is a method I have been performing for many years, and we have achieved very positive results with it.
Conclusions and Recommendations
Erectile dysfunction (ED) is much more common among men than is commonly believed and is a significant health problem that affects quality of life. However, when properly diagnosed, it is not an untreatable condition. It is crucial for men experiencing erectile problems to consult a specialist urologist without delay.
I am Prof. Dr. Volkan İzol, a urologist, and I have evaluated and treated many patients with this condition. If you are experiencing symptoms consistent with the causes of erectile dysfunction, it is possible to find a personalized solution using the latest methods.
Remember, this isn’t just your problem—it’s a problem for your relationship and your quality of life as well. Don’t put it off; take the right step.
