For men, the topic of “penis size” is an issue that directly affects both self-confidence and sex life—and is often blown out of proportion.
Some of the most common things I hear at the clinic are:

  • “Doctor, is my penis small, or is it normal?”
  • “My spouse doesn’t say anything, but it bothers me.”
  • “When I look in the mirror in the shower, it brings me down.”
  • “Is it possible to increase breast size through surgery, and how realistic is it?”

First of all, I’d like to emphasize the following:
The publicperception of “normal penis size” has been seriously distorted by pornography and social media.
Penises that are often described to me as “small”are, medically speaking,well within normal limitsand do not require surgery.

Nevertheless, in some patients:

  • It could actually be at the lower end of the normal range for penis size,
  • What we call a micropenis may be a penis that is significantly small,
  • Or they may be of average height but seek surgery for aesthetic reasons or to boost their self-confidence.

This is where “surgical penis enlargement”—that is, penis lengthening, thickening, and combined surgical techniques—comes into play. In this article, I will explain surgical penis enlargement in detail, including its advantages, limitations, risks, and realistic expectations:

  1. Let’s Talk About the Facts First: What Is a Normal Penis Size?

Before planning surgery, it is necessary to determine whether the patient actually needs the surgery.

According to scientific studies in general:

  • Average penis length during an erection: around 12–16 cm.
  • 11 cm and above is generally considerednormal.
  • A measurement between 7 and 11 cm may be considered “at the lower limit—borderline”; the individual’s personal and functional status is important.
  • If it is less than 7 cm, it may fall into themicropeniscategory.

We need to make this clear:
👉 Penis length is measuredwhen erect,starting from the base.
Many men experience unnecessary anxiety when looking at images of their penis ina flaccid state.

As Prof. Dr. Volkan İzol, I always make the following distinction in my clinical practice:

  • A man who is medically normal but has psychological anxiety,
  • A man whose condition is significantly below normal and who could benefit from surgery.

It is not appropriate to perform surgery on every small penis we see;
Surgical penis enlargement is aserious and irreversibleprocedure and is not performed on just anyone who requests it.

 

  1. What Is Surgical Penis Enlargement? What Is It Done For?

Surgical penis enlargement involves:

  • To correct the appearance when it is flaccid,
  • Using certain techniquesto slightly increase the length and/or girth of the penis during an erection,
  • To achieve functional improvement in cases of micropenis or developmental anomalies,
  • To boost the patient'sself-confidence,
  • To reduce discomfort while getting dressed and undressed

These are aesthetic and reconstructive urological surgeries performed for specific purposes.

Is it purely cosmetic?

No. In some patients:

  • Congenital developmental delay,
  • Hormonal problems in childhood,
  • Buried penis,
  • Obesity-related buried penis

There are cases like these, and for these patients, surgeryis not onlya cosmeticprocedure but alsoa functional and medical necessity.

 

  1. What Are the Surgical Methods for Penis Enlargement?

We can examine surgical penis enlargement under two main headings:

  1. Penis Lengthening Surgery
  2. Penis Thickening Surgery (Girth Enhancement)

In some cases, botharecombined.

4.1 Penis Lengthening Surgery (Ligamentolysis)

There is a suspensory ligament that attaches the penis to the anterior wall of the abdomen.
This ligament holds part of the penis inside the body and determines the angle of the erection.

The basic principle behind penis enlargement surgeryis as follows:

  • Some of these suspensory ligaments are surgically cut (ligamentolysis),
  • A portion of the penisis “released” outward through a V-Y plasty or similar incision in the pubic region,
  • As a result, the penis appearslonger andmoreflaccidwhen it is not erect.

🔹Realistic Expectations:

  • A flaccid penis typically lengthens by2–4 cm.
  • The increase in length during an erectionis more limited, usually around 1–2 cm, and may not even be that much in every patient.

🔹Advantage:

  • It makes a cosmetically satisfying difference, especially in men who are overweight, have a potbelly, or whose penis appears to be buried inside the body.

🔹Disadvantages and risks:

  • There may be a slight downward shift in the angle of the erection (a more horizontal angle).
  • If proper care is not taken, the results may not be as expected in overweight patients with short torsos.
  • Use of a penile traction device may be necessary after surgery.

4.2 Penis Thickening Surgeries (Circumference Augmentation)

For many men, “girth” is just as important as length.
There are several different methods available for surgical girth enhancement:

4.2.1 Fat Injection (Lipofilling)

  • Fat is harvested from the patient’s own body (abdomen, waist, thighs),
  • It undergoes special processing,
  • It is injected uniformly between the penile skin and the cavernous tissues.

🔹Benefits:

  • It is the patient's own tissue (autologous), so there is no risk of an allergic reaction.
  • At first, it results in a noticeable increase in thickness.

🔹Disadvantages:

  • Over time, some of it dissolves (especially during the first 6–12 months).
  • If the melting fat does not melt evenly, it can causeunevenness, a rippled appearance, or lumps.
  • An additional session may be necessary.

4.2.2 Dermal–Fat Grafts, Fascia, or Other Grafts

In some surgical procedures:

  • Subcutaneous fat grafts,
  • Dermal grafts,
  • Some synthetic materials

It is placed around the penis to achieve a more controlled and lasting increase in girth.

These methods require more advanced surgical experience.

4.3 Combined Surgery: Both Lengthening and Thickening

In particular:

  • Men whose penis length is at the lower end of the range,
  • A distinctly short, loose-looking,
  • At the same time, it has a thin rim

In men,lengthening and thickeningsurgery can be performedin a single session.

These types of joint ventures:

  • The surgery takes longer,
  • Tissue healing is more critical,
  • The risk of complications is relatively higher

These are surgical procedures. An experienced team and proper patient selection are very important.

  1. Let’s Not Confuse This with Non-Surgical Methods: Let’s Be Transparent

In my urology and sexual health practice, I always make it clear to my patients that:

  • Compared to the results achieved through surgery,
  • Fillers (hyaluronic acid, penile fillers)or
  • Vacuum-only and device-basedtreatments
    are not the same thing.

Surgical penis enlargement:

  • Irreversible,
  • Performed under general or spinal anesthesia,
  • Leaves a certain amount of a trace,
  • It is a serious undertaking that involves a recovery process.

Fillers and similar methods:

  • Generallyless invasive,
  • Recyclable or biodegradable,
  • These are particularly suitable options for expanding your network.

For many patients, it is possible to achieve satisfactory results without surgery,using fillers, ESWT, PRP, exosomes, and combination therapies.
For this reason, “surgical penis enlargement” is not the first option we recommend to every patient.

 

  1. Who Is a Good Candidate for Surgical Penis Enlargement?

Potential patient profile:

  • Men whose penis size is medically consideredtrulysmall or at the lower end of the range,
  • Those with micropenis or developmental issues,
  • Those who have not benefited from non-surgical methods in the past,
  • Who do not have body dysmorphic disorder,
  • Those with realistic expectations,
  • Patients who consent to the surgery and recovery process.

Groups I have carefully evaluated:

  • Only those who come under pressure from their partners,
  • People who feel inadequate after watching pornography but are medically normal,
  • People with serious self-esteem or obsessive-compulsive issues from a psychiatric perspective,
  • “Grow 10 cm,” those waiting for a “miracle.”

In this group,psychological support and accurate informationare oftenmore important than surgery.

  1. What Do I Discuss During the Pre-Surgery Consultation?

During my consultation with a patient planning to undergo surgical penis enlargement, I always discuss the following topics in detail:

  • What is the current penis length?(measured when erect)
  • Is it within normal anatomical limits?
  • Is the patient's main complaintthe appearance of the penis when flaccid, orits length when erect?
  • Is the quality of the erection sufficient?
  • Is there any excess weight (buried penis)?
  • Has he had any previous surgeries (hypospadias, trauma, etc.)?
  • How realistic are these expectations?

If surgery is deemed appropriate following this evaluation, I will provide clear information on the following points:

  • The expected average length/thickness,
  • The presence of traces,
  • The likelihood of a change in the angle of erection,
  • Risks of complications,
  • The challenges of the postoperative period.
  • What Are the Risks of Surgical Penis Enlargement?

Like any surgical procedure, penile enlargement surgery also carries risks, and I clearly explain these to my patients:

  • Infection
  • Suture line separation, wound healing problems
  • Asymmetry, deformity
  • Irregularities, nodules, or hardness in fat injection
  • Changes in his (especially in the dorsal region)
  • Downward shift in the angle of erection
  • A result that falls short of expectations leads to dissatisfaction
  • The need for additional corrective surgery, if necessary

Although these risks are low, they are serious—albeit to a small extent—because an “aesthetic and functional organ” is involved.

For this reason, the decision to undergo surgery mustbe madeafter careful consideration.

 

  1. The Postoperative Period: What to Expect?
  • Rest for 1–2 days is recommended.
  • During the first week, you may experience swelling, bruising, and mild pain.
  • It is important to keep the incision site clean and to change the dressings regularly.
  • Most patients return to their daily lives within a week.
  • You should generally wait 4–6 weeks before having sex (this may vary depending on the method).
  • In some cases, the use of a penile traction device is recommended (especially after penile lengthening surgery).

You’ll need to waitat least 3–6 monthsto see the final results of the surgery.
It takes time for the swelling to completely subside, for the tissues to settle, and for it to become clear how much fat tissue remains.

 

  1. How Long Do the Results of Surgery Last?
  • In limb-lengthening surgery, the change in neck lengthisusuallypermanent.
  • Changes in erectile length are more limited; in most cases, the permanent effect is maintained.
  • With fat injection for augmentation, however, some of the fat is absorbed over time; the remainder is permanent, and additional sessions may be necessary for some patients.
  • Graft-based techniques offer higher long-term success rates, but they involve greater surgical complexity and risk.
  1. Surgical Penis Enlargement Is Not a “Miracle”

I’d like to emphasize this point in particular:

👉 Surgical penis enlargement is not a procedure thatworks miracles or guarantees results of 20 cm or more for everyone.

This surgery:

  • It brings about a limited but meaningful change,
  • In particular, it can lead to a noticeable improvement in a sagging appearance and self-confidence,
  • It can be satisfactory in the right patient,
  • If it is administered to the wrong patient, however, it can lead to disappointment and regret.

For this reason, it is essential to providehonest, detailed, and realistic informationbefore the procedure.
